Appointments
How do I make an appointment?
Appointments require a referral from another doctor. This can be your primary care provider, another specialist, or a hospital. We need to see what care you've already received so that we can make the most accurate and effective plan of treatment for you.
Once we receive that, either the referring provider will make you an appointment, or we'll reach out to you to schedule you in.
What do I need for my appointment?
We need a bit of documentation in order to process your appointments:
- A photo ID
- All of your insurance cards
- A list of your current medications
When do I pay my co-pay?
Your co-pay is due at the time of your appointment. We can take cash, check, and we accept all major credit cards.
What do I do if I have a deductible?
We can check to see if you have reached your deductible amount. If not, we are required to take a deposit for your deductible at the time of your appointment.
Is there any paperwork?
We have a few forms for you to fill out and sign before your appointment:
- A form giving us your consent to be treated.
- A form regarding how you and your health insurance carrier will be billed.
- A short medical history questionnaire.
We recommend coming to the office 10-15 minutes before your scheduled appointment time so you can fill these out.
